Your continuing healthcare is a priority for us. Consequently, it is important that you establish contact with another physician as soon as possible. In the interim, if you need copies of recent medical records, lab or x-ray results, please contact us via one of the methods listed below. All medical records are confidential and protected by federal privacy and security regulations. Consequently, a completed and signed HIPAA medical records release form must be downloaded from our website and mailed, e-mailed or faxed to us. Upon receiving the completed forms, we will mail, fax or e-mail an invoice for the cost of mailing the last six months (unless you specify otherwise) of your medical records. Upon receiving payment, we will mail, fax or e-mail copies of your records. We will strive to send your records as soon as possible but please allow 30 days.
